Identify Your Request Type: Choose the button that best fits your current issue:
  • User Concern: For general software help, login issues, or "how-to" questions.
  • IT Technical Tickets: For physical hardware repairs, printer issues, or network connectivity.
  • Request Forms: Use these specifically for account deletions, building access permits, or new device requests.

Complete the Microsoft Form: After clicking, a new tab will open. Provide your Employee ID, Department, and a clear description of the problem. Attaching screenshots (if possible) helps us resolve tickets faster.

Submit & Monitor: Click "Submit" on the form. You will receive an automated email confirmation. Our team will review the ticket and reach out via Teams or Email based on the priority level.
